Evidence Guide
Logbook 50%
Clinic Supervisor's Reports 50%
Successful completion of both criteria is required for a PASS grade.
A logbook of observations and experiences will be maintained by the student and
delivered to the Course Supervisor on the final week.
Each supervisor will report, by way of completion of the "Student Appraisal
Form", on the practical ability and professionalism of the student.
A pass grade will only be given after a 'competent'
assessment grade is received in both the logbook and
Supervisor's reports.
(NOTE: One 'Not Competent' clinic supervisor's report will require
counselling by the Course Supervisor. If a further "Not Competent" report is
received then the student will be placed on "Hold").
