Description
This unit describes the performance outcomes, skills and knowledge required to provide supportive care of a person with renal impairment and advance care planning.This unit applies to enrolled nurses, registered with the Nursing and Midwifery Board of Australia (NMBA), seeking specialisation in enrolled nurse work that is carried out in consultation and collaboration with registered nurses, and under direct or indirect supervisory arrangements aligned to the NMBA regulatory authority legislative requirements.The skills in this unit must be applied in accordance with Commonwealth and State/Territory legislation, Australian Standards and industry codes of practice.No occupational licensing, certification or specific legislative requirements apply to this unit at the time of publication.

Elements and Performance Criteria
1. Identify a person’s knowledge of the impact of renal impairment.
- 1.1. Assess knowledge of renal impairment and impact on lifestyle, social context and emotional and spiritual choices.
- 1.2. Confirm understanding of the pathophysiology of renal impairment.
- 1.3. Identify common problems and complications associated with renal impairment and contributing factors that impact on health and lifestyle.
- 1.4. Identify psychosocial impacts of renal impairment on activities of daily living.
- 1.5. Provide support in a culturally safe and open manner.
- 1.6. Confirm spiritual and cultural issues related to the impacts of renal impairment.
2. Contribute to health status in renal impairment.
- 2.1. Perform primary health care assessment.
- 2.2. Monitor health status to identify renal impairment and its progression, and document and report.
- 2.3. Identify psychosocial impacts of renal impairment and refer for counselling and assistance.
- 2.4. Consult with the multidisciplinary health care team to contribute to care planning.
3. Contribute to provision of patient education in renal impairment.
- 3.1. Identify knowledge deficits of the renal impairment.
- 3.2. Provide information and resources on the aetiology and pathophysiology of the stages of renal impairment.
- 3.3. Apply contemporary information to assist optimal health status and slowing renal impairment progression.
- 3.4. Clarify educational requirements for the stages of renal impairment and apply to nursing plan of care and self-management.
- 3.5. Provide education and support to establish and maintain optimal nutrition and dietary choices in renal impairment.
- 3.6. Contribute to health education, resources on renal impairment and renal replacement therapy.
- 3.7. Communicate information about referral to multidisciplinary health care team for alternative care options as renal impairment progresses, including end of life care.
- 3.8. Contribute documentation to identify and meet advance care planning and directives.
4. Contribute to self-management strategies for renal impairment.
- 4.1. Identify opportunities for self-management of clinical manifestations and common complications associated with renal impairment.
- 4.2. Provide information and access to resources for self-care management strategies to maintain optimal health status.
