Description
This unit involves the skills and knowledge required to investigate an incident associated with railway operations in accordance with rail safety regulatory and workplace requirements.It includes planning the investigation, collecting and analysing information, and preparing the safety incident report.People achieving competence in this unit will need to comply with the relevant state/territory rail safety legislation, codes of practice, rail network standards and/or guidelines.Work is performed under some supervision generally within a team environment. No licensing, legislative or certification requirements apply to this unit at the time of publication.

Elements and Performance Criteria
1 Plan the investigation
  • 1.1 Clarification of the incident investigation objectives is established in conjunction with relevant personnel in accordance with workplace and regulatory requirements
  • 1.2 Investigation timeframes and locations are established in conjunction with relevant personnel that includes the investigation, draft report, recommendations and final report
  • 1.3 Site parameters, operational work systems, equipment, management and site operating systems for investigating safety incidents are determined in accordance with the scope of investigation
  • 1.4 Required investigation resources are identified and obtained in accordance with workplace procedures
2 Collect and analyse information
  • 2.1 Relevant available incident information is obtained and collated to facilitate a detailed and accurate incident analysis
  • 2.2 Interviews are conducted with relevant personnel to establish their recollection of events associated with the incident
  • 2.3 Information is analysed in accordance with the established investigation objectives, based on approved conditions, applicable rail network standards and operational guidelines
  • 2.4 Analysis criteria are identified to enable evaluation of the findings in accordance with the scope of investigation
  • 2.5 Options that lead to recommendations that reduce future risk are identified and corrective actions generated in accordance with workplace and/or statutory requirements
3 Summarise and report on progress of investigation
  • 3.1 Ongoing monitoring of the investigation, changes in direction and resource implications are recorded and reported
  • 3.2 Progress, problems and resource usage are reviewed in accordance with the investigation scope
  • 3.3 Progress reports and/or briefings are prepared and disseminated in accordance with workplace and/or statutory requirements
  • 3.4 Records and systems are secured and updated regularly in accordance with workplace and/or statutory requirements
4 Prepare report
  • 4.1 Contributing factors are identified and recommendations are made that will enable a satisfactory resolution of the incident issues, and meet workplace and statutory requirements
  • 4.2 Opportunities to enhance operational efficiency and safety procedures are documented in accordance with workplace reporting guidelines
  • 4.3 Draft incident report is distributed to relevant stakeholders for validation in accordance with workplace and/or statutory requirements
